Condition: Note that piece 3, the casing piece is missing. Other than that, pattern is cut and complete. The two short sleeve pieces have writing on them ("sew this end to dress"). No envelope. Manufacturer is not indicated.

Dress can be made with long or short sleeves - the short sleeves have a "tulip" shape.
